Membership & Pricing
Membership is priced by age: $99/month for ages 3–17, $149/month for ages 18–29, and $199/month for ages 30 and older. Household savings may be available. Current rates are always shown on the Membership page.
Full Circle Access includes routine primary care visits, an annual preventive exam, acute care, chronic condition care, secure messaging, office testing and procedures, medication management, lab access, sexual health and PrEP, mental health, weight and metabolic health, and LGBTQ+ inclusive care. A total of 12 in-person and virtual visits are included each year.
Yes. A one-time enrollment fee of $50 applies when you join, with a maximum of $150 per household.
Yes. After the first three months, membership is month to month. You can cancel at any time with at least one week's notice before your next billing date by emailing the office or sending a message through the patient app.
No. There is no long-term commitment past the first three months. After that, membership is month to month and you can cancel with at least one week's notice before your next billing date.
Most primary care is included. Outside laboratory testing, imaging, medications, and some procedures, supplies, or lab fees may cost extra. Expected costs are discussed in advance whenever possible.
Appointments & Access
Members can request or book visits through the SigmaMD patient app or by contacting the office. New patients start by scheduling a complimentary Meet & Greet.
Same- or next-business-day visits are often available for urgent concerns, but we cannot guarantee it. The practice will help find the best time and type of care based on your symptoms.
Yes. Telehealth is available when it is medically appropriate. Because Dr. Venis is licensed in Indiana, you must be physically in Indiana at the time of a telehealth visit that involves diagnosis, treatment, or prescribing.
Members can message the practice through the secure SigmaMD patient app. Messages are checked during business hours, and response times depend on urgency and clinic volume.
For a life-threatening emergency, call 911 or go to the nearest emergency department. For urgent concerns that cannot safely wait until the next business day, current members may use the after-hours phone option. Routine questions, refills, scheduling, and test-result questions should go through the patient app or regular clinic voicemail.

We aim to respond quickly when Dr. Venis is available, but after-hours responses are not guaranteed. If you need care right away or do not get a quick response, go to urgent care or the emergency department. For a life-threatening emergency, call 911.
Insurance & Outside Care
INcircle Health does not bill insurance for membership fees or routine care through the membership. Membership is not health insurance. Members can still use insurance for outside services like labs, imaging, specialists, and hospital care.
Yes. Members can usually use insurance for lab testing, imaging, medications, specialist care, and hospital services. Coverage and out-of-pocket costs depend on your insurance plan.
Membership is not health insurance and does not cover emergencies, hospital stays, surgery, or specialist care. We recommend maintaining insurance or another form of major medical coverage for those needs.
The practice can recommend and refer you to specialists and help coordinate care. The specialist visit is not included in membership and is billed separately by the specialist or facility, often through your insurance.
Yes. The practice can coordinate with specialists, pharmacists, therapists, and other clinicians while maintaining continuity in your primary care.
Labs, Medications & Procedures
In-office sample collection (such as a blood draw) is available for a $10 fee. Most outside laboratory processing — for example through Quest Diagnostics — is separate from membership and may be billed to insurance or paid at clear cash-pay prices.
Selected point-of-care testing, blood draws and specimen collection, electrocardiograms (EKG), and other routine in-office testing can be done at the clinic. Outside lab processing is separate.
Many minor in-office procedures are included, such as skin biopsies, cryotherapy, joint injections, abscess drainage, stitches, and other routine procedures. Some procedures, medications, supplies, or lab fees may cost extra and are discussed in advance.
Yes. Members have access to low-cost generic medications and clear cash-pay pricing for many common lab tests through Quest Diagnostics. The practice can also help you explore affordable options, including lower-cost alternatives when insurance does not cover a medication.
Yes. For outside services such as lab testing, imaging, and medications, you can usually use your insurance. Coverage and out-of-pocket costs depend on your plan.
Children & Families
INcircle Health cares for patients age 3 and older. Virtual Access membership is for adults age 18 and older.
Yes. Children age 3 and older may join, depending on current availability. Pediatric well visits and sports physicals are part of primary care.
Yes. Children and adults may join, and household savings or enrollment-fee limits may be available. Current details are explained on the Membership page.
Yes. When clinically appropriate, parents and children can be seen together. A parent or guardian does not have to join for a child to become a member, though a family membership can make coordination easier.
Vaccines are not usually given on site unless you obtain the vaccine from a pharmacy and bring it to the office. We direct patients to low-cost and free vaccination clinics, or you may get vaccines at a pharmacy using your insurance.
Privacy & Communication
Your health information is protected under applicable privacy laws, including HIPAA. Information is shared only as permitted or required by law and as needed for your care.
Yes. Members can message Dr. Venis through the secure SigmaMD patient app, and a web portal is also available. Secure messaging is checked during business hours.

Sensitive concerns — including sexual health, mental health, and gender-affirming care — are discussed respectfully and privately. You do not have to share more than you are comfortable with, and your information is handled with care.
Joining INcircle Health
Start by scheduling a complimentary Meet & Greet. It is a short, no-pressure conversation to learn how membership works, ask questions, and see if the practice is a good fit before you join.
The Meet & Greet is an introductory chat. It does not include a medical exam, diagnosis, prescriptions, or treatment, and it does not establish a physician-patient relationship.
We recommend it. The Meet & Greet lets you meet Dr. Venis, understand the membership, and decide whether the practice is right for you before enrolling.
New members receive an invitation code for the SigmaMD patient app, where you can message Dr. Venis, schedule visits, view records, and manage your membership. The Current Member Guide walks through everything you need to get started.
If you have recent records — including past medical history, medications, allergies, immunizations, and lab or imaging results — sharing them helps Dr. Venis understand your background. You can request records from your previous provider or upload them through the patient app.
