Medical exams for immigration.
INcircle Health offers immigration medical exams required by USCIS. They are done by a USCIS-designated civil surgeon who is also a board-certified family physician, in a respectful, private, and welcoming setting.
USCIS medical evaluations
Immigration Medical Examination
Required by U.S. Citizenship and Immigration Services (USCIS) for people applying for lawful permanent residency (a green card). Done by a USCIS-designated civil surgeon.
$375 total
-$50 non-refundable deposit, due at scheduling

- Review of immigration paperwork and medical history
- USCIS-required physical examination
- Vaccination record review and immunization determination
- Ordering and coordination of all required laboratory testing
- Completion of USCIS Form I-693 by a designated civil surgeon
- Sealing of the completed I-693 form for USCIS submission
- Clear instructions on next steps and document handling
Medical Certification for Disability Exceptions
Lets people applying for U.S. citizenship ask to skip the English and/or civics test because of a qualifying medical condition. It includes a detailed medical history review, a thinking or daily-living check when needed, and completion of the N-648 form if you meet the rules.
$450 total

- Comprehensive review of medical, psychiatric, and functional history
- Cognitive or functional assessment when clinically appropriate
- Review of supporting medical records (when available)
- Determination of whether USCIS criteria for the N-648 are met
- Accurate completion of USCIS Form N-648 if eligibility criteria are satisfied
Certified Interpreter Services — $100
Dr. Venis speaks Spanish fluently, so Spanish-speaking patients do not need an interpreter. For other languages, USCIS requires a certified interpreter. You may also bring your own certified interpreter, as long as there is no conflict of interest and they meet USCIS rules. Please tell us ahead of time if you plan to bring your own interpreter or will need to use one of ours.
What you need to know about vaccines
Vaccines are not included in the exam price and are not given on site. If you need more vaccines, we help you get them at low or no cost — especially if you don't have insurance — through pharmacies, the health department, or community clinics. Please bring all vaccination records you have to your appointment. Patients must bring certified English translations of any official vaccination records from their home country.
N-648 Scheduling Process
To schedule a Form N-648 evaluation, contact us directly to arrange your appointment. Unlike the I-693, the N-648 is not scheduled online.
I-693 Scheduling Process
USCIS exam availability is limited because primary care appointments remain our scheduling priority. To schedule multiple family members or request a time not listed online, call 317-676-6498.

Choose a date and time, then pay the $50 deposit to confirm. The remaining $325 is due at your appointment.
